Treating neck arthritis: what can actually be changed
Patients diagnosed with cervical spondylosis often hear that it is irreversible and conclude treatment is pointless. The structural change is indeed irreversible, but most symptoms arise not from the structure but from the muscular and joint balance carrying it.

Regaining range of motion
Mobilisation of stiffened cervical segments prevents movement concentrating at particular levels. As load spreads across several segments, the overloading that produces pain reduces.
Thoracic mobility is opened in the same period. When the upper back stiffens, more movement is demanded of the neck; the two regions are always worked together.
Range is measured in six directions and repeated each session. Progress is judged by measurement rather than by how the patient feels.
Building muscular support
Endurance training of the deep neck flexors is the core of the programme. When these muscles carry the head, load on the spondylotic segments falls.
Strengthening the scapular stabilisers corrects shoulder girdle position and reduces constant muscular tension in the neck.
Work is done at low load for long durations. The target here is endurance rather than strength; adding weight lets the superficial muscles dominate.
Realistic expectations and timeline
The programme's aim is not to change the X-ray finding. It is to reduce the intensity and frequency of pain, increase range and preserve daily function.
A clear reduction in symptoms is usually seen within four to six weeks. The programme typically runs 8–12 sessions and continues with home exercise.
As spondylosis is permanent, the programme is also repeated periodically. A few weeks of regular work each year is enough for most patients to maintain gains.
Key points
- The structural change is irreversible; pain and function are not.
- A neck programme is incomplete without thoracic mobility work.
- Deep neck flexor endurance is the core of the programme.
- The target is endurance, not strength; load is not increased.
- The programme is repeated periodically.
Frequently asked questions
Do electrotherapy devices help?
They can contribute briefly to pain management. As they do not change range or muscular endurance, however, they give no lasting result applied alone.
Is massage helpful in neck arthritis?
It temporarily reduces tension and is comfortable. As it does not build load-carrying endurance, tension returns within days; it is valuable as a component but insufficient alone.
Should I apply heat?
Warmth is comforting for morning stiffness and muscle tension, and used before exercise it makes movement easier. Fifteen to twenty minutes, a few times a day, is appropriate.
