Pillow, screen and daily routine in neck arthritis
In cervical spondylosis the severity of symptoms depends markedly on daily habits. The same structural picture can cause very different degrees of discomfort as sleep and working position change.

Pillow and sleep
A pillow should be high enough to fill the shoulder width when side-lying. Too low bends the neck sideways, too high pushes it forward — both increase morning stiffness.
A thinner pillow is needed when supine. For those using both positions, a medium-height pillow that holds its shape is suitable.
Sleeping face down holds the neck fully rotated for hours. In spondylosis this markedly increases morning symptoms and gradually giving it up is advised.
The morning routine
Morning stiffness is an expected finding here and usually resolves within thirty minutes. A warm shower shortens that time.
A short range-of-motion session after the shower markedly reduces stiffness through the rest of the day. Five minutes suffices for most patients.
Rolling to the side and pushing up through the arms, rather than sitting straight up, reduces the sudden load on the neck.
Working setup
The top of the screen should be at eye level, directly in front. A low screen increases load on the spondylotic segments all day.
The arms should be supported. Unsupported arms keep the shoulder girdle working and increase tension in the neck.
Changing position every forty-five to sixty minutes is more effective than the best ergonomic setup. Interrupting static load is decisive here.
Key points
- A pillow should fill the shoulder width when side-lying.
- Sleeping face down markedly increases morning symptoms.
- Morning stiffness usually resolves within thirty minutes.
- Five minutes of range work after a shower makes a difference all day.
- How often you change position matters more than the ergonomic setup.
Frequently asked questions
Is an orthopaedic pillow essential?
Not essential. What matters is not the brand but whether the height suits your sleeping position. An ordinary pillow at the right height works just as well.
Should I use a collar?
Routine use is not advised. A collar reduces muscle work and leads to loss of endurance over time, which increases symptoms in spondylosis.
Does weather change increase pain?
Many patients report increased symptoms in cold, damp weather. The mechanism is not fully explained; keeping the area warm and moving regularly helps in those periods.
