Is cervical flattening serious? When assessment is needed
Most patients who see 'flattening' on a report interpret it as serious damage. The finding itself is not serious; what matters is the accompanying symptoms. Below, the expected picture is separated from the one requiring assessment.

The expected picture, needing no concern
Neck and shoulder tension that builds through the day, peaks in the evening and eases with rest is the typical load-related picture. It indicates muscular endurance is not meeting daily demand and improves with exercise.
Bilateral, pressure-type headache starting at the base of the skull and spreading to the temple also belongs here. Neck-related tension headache is common and recedes alongside a neck programme.
Clicking when turning the neck is not significant on its own. Without accompanying pain and restriction it is a normal event at the joint surfaces.
Symptoms requiring assessment
Pain radiating into the arm, below the elbow, with numbness in the fingers suggests nerve root compression. This picture requires assessment independent of the flattening.
Loss of grip strength or dropping objects should be assessed without delay. Objective reduction in muscle power can indicate progressing nerve compression.
Unsteadiness when walking, stiffness in the legs, or changes in bladder or bowel control require urgent assessment. These raise the possibility of compression at spinal cord level and are not suitable for watchful waiting.
When is imaging needed?
In patients with purely mechanical neck pain and no neurological signs, advanced imaging usually does not change the treatment decision. It contributes nothing beyond unnecessary anxiety.
MRI becomes meaningful where there are arm symptoms, loss of power, or a picture not improving despite six weeks of appropriate treatment. Absent those three criteria, waiting is preferred.
With fever, unexplained weight loss, a history of cancer, or pain after significant trauma, the imaging decision is taken without waiting.
Key points
- Cervical flattening alone is not a serious finding.
- Seriousness is determined by accompanying neurological symptoms.
- Numbness radiating into the arm and loss of power require assessment.
- Unsteady walking or changes in bladder control are urgent.
- Without neurological signs, imaging usually does not change treatment.
Frequently asked questions
Does flattening progress?
In adults the angle usually stays stable. Prolonged forward-head posture can increase it somewhat; muscular support and changes in position halt that tendency.
Can it cause dizziness?
Cervicogenic dizziness is possible, but dizziness has many other causes. It should not be attributed to the neck before inner-ear and blood-pressure causes are excluded.
When should I see a physiotherapist?
Assessment is appropriate for neck pain lasting more than two weeks that affects daily tasks or sleep. With numbness radiating into the arm or loss of power, seek assessment without waiting.
