Managing neck and shoulder tension through the day
In myofascial neck pain, trigger points resolve with treatment but re-form when daily load is unchanged. The adjustments below directly target the continuous load on the muscle and matter as much as the treatment.

Arm support and working setup
Without arm support, the shoulder girdle muscles carry the arm's weight all day. A chair with armrests, or sitting close enough to rest the elbows, is the single most effective change.
The top of the screen should be at eye level and directly in front. Working turned to one side loads one side of the neck and concentrates trigger points there.
Mouse and keyboard should sit close to the body. Reaching out draws the shoulder forward and up, keeping the upper trapezius continuously active.
Phone, bags and breaks
Cradling a phone between shoulder and ear is a classic habit that overloads the levator scapulae. A headset removes that load entirely.
Carrying a heavy bag on one shoulder keeps the upper trapezius contracted. A two-strap backpack is preferred; if one shoulder is used, alternate sides regularly.
Shoulder circles and neck movements every forty-five to sixty minutes interrupt static load. These short breaks are markedly more effective than one long one.
Sleep, stress and general routine
Pillow height should fill the shoulder width when side-lying. An unsuitable pillow bends the neck sideways all night and increases trigger point tenderness in the morning.
Under stress the neck and shoulder muscles tighten reflexively. That tension goes unnoticed and lasts hours; conscious shoulder-relaxation reminders through the day help.
Regular aerobic activity reduces muscle tension and raises the pain threshold. Thirty minutes of brisk walking three times a week is a sufficient starting point.
Key points
- Unsupported arms keep the upper trapezius working all day.
- Working turned to one side concentrates trigger points on that side.
- Cradling a phone at the shoulder overloads the levator scapulae.
- An unsuitable pillow increases morning tenderness.
- Stress-related muscle tension goes unnoticed and lasts hours.
Frequently asked questions
Does a hot shower help?
It does. Heat reduces muscle tension and makes stretching easier. Stretching after a shower is more effective than on a cold muscle.
Is an orthopaedic pillow essential?
Not essential. What matters is not the brand but whether the height suits your sleeping position. An ordinary pillow at the right height works just as well.
Which gym movements should I avoid?
Shrugs and heavy overhead movements can increase symptoms where the upper trapezius is already fatigued. Back movements that draw the shoulder blade backward are beneficial here.
